Job Description

Main purpose and scope of the job:


  • Provide individual domestic support to people in their own homes to promote independence and social inclusion
  • Provide support as part of an agreed HomeLife plan which can include accompanied visits outside of the home.
  • Help customers  to  prevent deterioration in their quality of life and the need for health and social care intervention.
  • Take an active role in the development and promotion of the scheme to people living within the community and with other agencies.
  • Be an effective and supportive member of the HomeLife team


Principal Duties and Responsibilities: 


To provide HomeLife service to designated individuals by:

  1. Assessing the support needs with each customer and drawing up a mutually agreed plan. Reviewing the plan regularly in consultation with customer and HomeLife Manager.
  2. Visiting each customer regularly to provide appropriate support as agreed in support plan.
  3. Tasks within the HomeLife plan will include the following:


In The Home- helping with:

  • Cooking
  • Cleaning
  • Ironing
  • Laundry tasks
  • Help with moving home- all practical tasks eg packing/cleaning/arranging viewing etc
  • Other domestic tasks as requested by the customer


Correspondence and Appointments:

  • Reading letters and bills to customers and helping them to understand/read the contents and deal with any requirements
  • Paying bills- helping people to get to post office/bank/make arrangements for paying rent etc
  • Arranging appointments- eg GPs, arranging for hospital transport, etc
  • Helping customer to reply to any correspondence including use of I.T


Accompanied Visits

  • Helping with shopping trips
  • Health appointments
  • Going with customer for a walk or helping customer to access exercise and leisure facilities
  • Going with customer on a trip or outing


Rebuilding Confidence

  • Assisting customer after they have come out of hospital, and helping them to manage in their home following hospital stays
  • Helping customer to get out and about generally and giving encouragement
  • Helping customer cope with an illness or disability
  • Enable customer to make new friends and social contacts by being aware of all local activities and networking effectively with other locality agencies and events
  1. Maintain appropriate records and administration of the project.
  2. Liaise regularly with support officers in your locality and attend team meetings and training as required. Report on a weekly basis to the HomeLife Manager to review and discuss support plan of needs for each tenant.
  3. Uphold the Equalities and Diversity Policy of the Group
  4. Take responsibility for the health and safety of oneself when visiting customers in their homes.
  5. To undertake any other duties that may be required from time to time commensurate with the grade and general nature of the post
  6. To provide cover for HomeLife team members in instances such as holidays and sickness.

Special Conditions

  • The post holder will be based in a support hub area and will claim mileage to travel to customers from the hub. Additional hours will be added according to demand from customers and availability of post holder.
  • The post may require work outside normal working hours, therefore a flexible working pattern within the hours for the post will be necessary.
  • Will often be working in a lone worker capacity.
  • May be subjected to unexpected abusive and unpleasant behaviour

Candidate Requirements

Person Specification




Experience of working with older or vulnerable people in their own homes.

Understanding or awareness of working with people who have hearing, sight or mobility loss.

Knowledge of a variety of vulnerabilities and acute medical conditions








Qualifications – good general level of qualifications.


English & Mathematics at GCSE level or equivalent.]


NVQ 2 Promoting Independence or Care or equivalent


An understanding of promoting independent living to include coaching and prompting skills.


Knowledge of the protection of vulnerable adults process.


Knowledge of cleaning in a home environment.


Understanding of health and safety issues in the home.


Knowledge of Food Hygiene procedures.


Manual Handling qualification.

he ability to communicate with, and motivate, vulnerable people.


Have clear and effective written and verbal communication skills


Must show a keenness to train towards NVQ 2 related qualification if appropriate.


Must have a valid driving licence and access to a vehicle for business use.


Must have business use car insurance in order to carry passengers.


































Must demonstrate a keenness to learn new skills associated with the post.


Can work on own initiative and be flexible to respond to the needs of the service.


Demonstrate an ability to work with other professionals.


Demonstrate an ability to be an effective team member.
